We follow the state mandated forms for signature plus the addition of a home warranty which must either be accepted or waived by the client. If a client refuses to sign a mandatory form which is not part of the contract itself (agency disclosure, lead paint, home warranty waiver), then the agent can return the document with a box checked that the form was provided and the client refused to sign.

If someone refuses to sign a state disclosure we merely write across it "buyer refuses to sign" and submit it with everything else.

We have only one non-state-required disclosure, which discloses we are partial owners of a title insurance company. Federal law requires that we disclose and I've never had a client refuse to sign a disclosure. Afterall, that's all is--a disclosure, not a contract.
