When you don't look like your photo anymore is a good clue. When your attire or hair style looks like your photo was taken in the last decade. When your dog is in the photo with you and it's not your partner in business...and I love dogs, but please! When the photo looks like you're selling more than real estate. I'll end it here.

John - I believe you should change your photo to reflect what you currently look like. I have some agent business cards, and then when I have met them in person they are like 20 years older, and barely even look like the same person. I would say every couple of years.

Looks like you got a new headshot... cool. I have that on my list of things to do in the next couple of months because my hair is longer now (though I may trim a couple of inches off before having the photos taken) and it's been several years since I've updated the photo.
