I get irritated by these when I see them, leaves the client unrepresented and leaves the other agent with a mess to clean up much of the time.

Flat Fee MLS, at least here in Florida, refers to MLS entry only for a fee. The Seller handles all calls, appointments, showings, offer presentation, etc. A true Flat Fee MLS Broker would have the Seller sign a No Brokerage Relationship statement (pursuant to Florida Law).
Some Brokers offer "menu-based" services or "a la carte". For example, put it into MLS, rent them a Supra Lockbox for an additional fee, set up showing appointments for a fee, and other service choices. (One thing a Florida Broker cannot / should not do is charge a fee to "review" offers or give guidance concerning contracts. This may constitute unauthorized practice of law).
One additional comment... I warn Sellers that unless they are investors or dealers who buy and sell a lot of houses, the Flat Fee MLS may not be a good idea. You the Seller may be frowned upon by Real Estate Agents, the Buyers may treat you like a FSBO. Also, you the Seller should seek Legal Advice from an Attorney if you are attempting to sell your own house using Flat Fee MLS.
